Waiver
Every player – and parent or guardian, if player is under 18 – must read this waiver. Electronic Signature on the registration form signify each person has read, and fully understands and abides by this information. There are risks associated and connected with my participation in this tournament and its related activities. I release and discharge Kosair Charities, Event Charities, Sponsors, Organizers, Louisville Hoopfest, Louis V LLC, and Kosair event workers, volunteers, employees and directors from all action, suits and demands whatsoever in law or in equity including but not limited to, the risk of injury from playing in the tournament and the risk of loss of personal property by theft or otherwise. Futhermore, I hereby grant full premission for event organizers to record any or all of my participation in this event for photos, motion pictures, TV, radio, recording, videotapes, and other media known or unknown, and to use them, no matter by who taken, in any matter for publicity, promotions, advertising, trade or commercial purposes, without any reimbursement of any kind due to me, or the need to pay me any fee.
Play at your own risk. Louis V LLC, Kosair, Louisville Hoopfest, Sponsors, Partners, volunteer and staff, and anyone associated with Louisville Hoopfest will not be held responsible or liable for any injuries or loss before, during, or after the event.
